Thrombophlébites pelviennes du post-partum : diagnostic, traitement et suivi. Etude rétrospective sur 10 ans - 30/07/22

Introduction |
La thrombophlébite de la veine ovarienne du post-partum (TVOP) est une complication rare mais grave du post-partum pouvant mettre en jeu le pronostic maternel par ses risques embolique et septique. Le diagnostic clinique et paraclinique est difficile du fait des signes peu spécifiques et de l’absence de « gold standard » pour l’imagerie. Il n’y a pas de consensus dans la littérature ni sur le traitement ni sur le suivi de ces patientes. L’objectif principal était de préciser les signes cliniques et paracliniques évocateurs d’une TVOP dans le but d’améliorer le délai diagnostique. Les objectifs secondaires étaient de décrire l’étendue de la TVOP et la prise en charge thérapeutique immédiate proposée.
Méthodes |
Il s’agit d’une étude rétrospective sur 10ans dans une maternité de type III, entre janvier 2010 et décembre 2019, où toutes les patientes avec un diagnostic de TVOP confirmé à l’imagerie étaient incluses. Nous avons analysé les données cliniques, paracliniques et le suivi des patientes.
Résultats |
Nous avons inclus 9 patientes avec une confirmation diagnostique par imagerie. Le délai moyen entre les premiers symptômes et le diagnostic étaient de 3,3jours (±3,5jours) et le diagnostic de TVOP n’avait été évoqué que pour 2 patientes (22,2 %) avant l’imagerie. Toutes les patientes ont bénéficié d’une anticoagulation curative et 77,8 % (n=7) ont reçu une antibiothérapie pour la TVOP. Deux patientes avaient une forme compliquée, une par une embolie pulmonaire et une d’une compression des voies urinaires ayant nécessité une dérivation urinaire par une sonde double J. Cinq patientes (55,6 %) ont réalisé un bilan de thrombophilie.
Conclusion |
Le diagnostic de TVOP est difficile et nécessite d’être évoqué devant une symptomatologie douloureuse ou une fièvre en post-partum. Il peut être réalisé par échographie, mais la tomodensitométrie injectée précisant la recherche spécifique d’une TVOP reste l’examen d’imagerie de choix afin d’affirmer le diagnostic et d’éliminer les diagnostics différentiels. Sous anticoagulation curative et une antibiothérapie à large spectre, l’évolution clinique est généralement très favorable. Une consultation par un médecin spécialiste d’organe permet de définir des consignes pour une grossesse ultérieure.

Introduction |
Postpartum ovarian vein thrombosis (POVT) is a rare but serious postpartum complication that can be life-threatening due to its embolic and septic risks. The clinical and paraclinical diagnosis is difficult because of the non-specific signs and the absence of a gold standard for imaging. There is no consensus in the literature on the treatment and follow-up of these patients. The primary objective was to specify the clinical and paraclinical signs suggestive of POVT in order to improve the diagnostic delay. The secondary objectives were to describe the extent of POVT and the proposed immediate therapeutic management.
Methods |
This was a 10-year retrospective study in a type III maternity hospital, from January 2010 to December 2019, where all patients with an imaging-confirmed diagnosis of POVT were included. We analysed the clinical and paraclinical data and the follow-up of the patients.
Results |
We included 9 patients with a diagnostic confirmation by imaging. The mean time from first symptoms to diagnosis was 3.3 days (±3.5 days), and only 2 patients (22.2 %) had been diagnosed with POVT before imaging. All patients received curative anticoagulation and 77.8 % (n=7) received antibiotic therapy for POVT. Two patients had a complicated form, 1 with a pulmonary embolism and 1 with a urinary tract compression requiring a urinary diversion with a double J catheter. Five patients (55.6 %) had a thrombophilia check-up.
Conclusion |
The diagnosis of POVT is difficult and needs to be evoked in front of a painful symptomatology or a fever in postpartum. It can be made by ultrasound, but the injected CT scan specifying the specific search for a POVT remains the imaging examination of choice in order to confirm the diagnosis and eliminate differential diagnoses. Under curative anticoagulation and broad-spectrum antibiotic therapy, the clinical course is generally very favourable. A consultation with an internist makes it possible to define instructions for a subsequent pregnancy.
